• Enrollment Agreement
• Authorization and Release Forms (3)
• Annual Commitment Fee of $400
• Soaring Wings Parent/Teacher/Student Organization Membership Form

You may drop these items off any school day or mail them to POB 682384, Park City, Utah 84068

When all of these items have been received and processed, you will receive confirmation of your child's enrollment.
